The Power of an Efficient Referral Network

In mid-August, the Crossings North Penn clinic referred a patient to HAU after a colposcopy revealed a high-grade squamous intraepithelial lesion (HSIL)—abnormal cells that can turn into cancer if left untreated.

A Referral Specialist quickly prepared the case, allowing HAU to immediately coordinate with a doctor at INTEGRIS Health. The patient was seen within days and recommended for a LEEP procedure to remove the cells. This is a classic example of our referral network working efficiently and effectively to deliver specialized care.

Relocating Vital Prescriptions

When the St. Charles-McAuley Free Clinic closed, HAU received an urgent call asking for help relocating the medications left in their pharmacy. Because our team was already actively working to transfer St. Charles’ Prescription Assistance Program (PAP) patients to Ally Clinic, we were perfectly positioned to solve the problem. We quickly connected the staff at both clinics, which allowed Ally Clinic to securely take in and store the remaining medications in full compliance with state pharmacy regulations.

Empowering Healthy Choices in the Healthy Living with Diabetes Program

For 20 years, an Oklahoma woman had been living with prediabetes and had been warned by doctors about the risk of developing type 2 diabetes. She had taken classes before to lose weight, but no one had ever explained to her that weight loss directly helps control blood sugar.

At 54 years old, she participated in our Healthy Living with Diabetes program. Previously, she did not fully realize how serious diabetes could be, as family members living with the condition seemed okay. Through this program, she learned that diabetes can severely impact many parts of the body, gaining a clear understanding of what it is, its types, and how it can lead to major health complications by damaging the heart, kidneys, eyes, and other vital organs.

The classes empowered participants to make healthy choices, with a focus on practical nutrition information. Together, she and her classmates learned what foods to eat, how to count calories, how to measure food, and the importance of avoiding immediate second portions.

She has only good things to say about the program, sharing that the instructor explained concepts in a clear, simple way, bringing a kind, fun approach that helps participants feel strong and ready to make a change. Feeling incredibly thankful to have joined, she now knows exactly what to do to keep her blood sugar at a healthy level.

She is committed to continuing her self-care to prevent type 2 diabetes, expressing deep gratitude to everyone who supports this program for helping people learn how to stay healthy.

Restoring Mobility Through Care Connection Referrals

For years, severe leg pain left a 35-year-old construction worker and father of three confined to a wheelchair. The toll was physical, emotional, and financial, as he was unable to work or participate in activities with his children. Thinking he might never walk again, the patient connected with HAU. In 2022, he received a life-changing, no-cost bilateral hip replacement.

Today, he is back to building schools and hospitals, and finally enjoying active time with his family. To show his heartfelt appreciation to everyone who made his care possible, he even delivered a basket of baked goods!

A Life-Changing Colostomy Reversal Through a Care Connection Referral

After a successful abdominal surgery left an uninsured man with a colostomy, he was unable to afford the necessary reversal procedure or even the daily supplies to manage it. Feeling desperate, he was referred to HAU.

Our Care Connection team connected him with a partner clinic for immediate supplies and specialized nursing care. Then, we reached out to the surgeon who performed the initial procedure. Not only did the surgeon agree to perform the life-changing corrective surgery at no cost, but they were so inspired by the collaboration that they pledged to donate one free service to HAU every month.
