Bridging the Gap for Chronic Condition Treatment

Before becoming a patient at Arubah Community Clinic, one Oklahoma man’s diabetes had been untreated for several years.

Through HAU’s support, Arubah helped him obtain free insulin and other oral medications to control his diabetes and blood pressure, effectively lowering his A1C levels. Through additional referrals, he also received diabetic wound care from a surgeon and podiatrist.

Navigating Care and New Community Connections

A bilingual navigator received a call from a patient who had moved to Oklahoma several years before and had not visited a physician since relocating.

The navigator referred her to Crossings Clinic; afterward, she expressed gratitude for the excellent care she received there, sharing that it was the miracle she had been praying for. She and her husband established Crossings as their primary care home, and even started attending the clinic’s church services.

Providing a Safety Net After Coverage Loss

HAU received a call from a patient who was discharged from her Medicaid plan and was unable to afford any Marketplace plan.

HAU connected her with a free clinic, where she had an appointment within a week and received critical medications she had been unable to obtain since losing coverage. In addition to physical support, the clinical director made sure she was taken care of emotionally.
